You have the choice of 3 levels of insurance coverage to choose from consisting of AT&T Mobile Insurance, Mobile Protection Pack, and Multi-Device Protection Pack.

These device protection plans are meant to protect your devices against loss, damage, theft and other malfunctions. If you have a broken AT&T U-Verse phone and have insurance, you should file a claim with AT&T.

However, before you file a claim, there are a few details that you should clarify. First and foremost, you should ensure that your device is covered by one of AT&T's device protection plans and you should know which on.

You should also ensure that you file your claim within 60 days of your device being damaged. There are also certain pieces of information that you should have ready before you file a claim including device brand and model, wireless security code or the last 4 digits of your social security number or tax ID number. You will also need proof that you are the owner of the device which may include a receipt, an affidavit or government issued identification.

File a Claim

You can only file 2 claims in 12 consecutive months for Mobile Insurance, 3 claims in 12 consecutive months for Protection for 1 and 8 claims in 12 consecutive months for Protection for 4.

All the above insurance policies are provided by Asurion. They use express delivery and will deliver a replacement in one or two business days.

The maximum value of each claim is $2,500. To file the claim, you will have to log on to the AT&T phone claim page and input your device model, zip code and AT&T insurance program to check your eligibility.

If you are eligible to receive a replacement for your broken phone, you will receive instructions as to how you can receive it. You can even give Asurion a call if your device is not working when you receive it.

With a wireless number of the affected device or a number associated with your AT&T account, you can track your claim until resolved and replacement device until it arrives at your doorstep.
